About the Position

We currently have vacancies within our organisation in our community options at our Frankston & Mornington sites. The position is in our Connect 2 Community program focused on the integration of adults with a disability into the community.

As a Facilitator, you are offered the chance to work closely with our customers to genuinely understand their goals and then to assist them to achieve meaningful outcomes. You will see the potential rather than the limitations in people and will be committed to the achievement of real and meaningful outcomes.

About the Person

You are mature-minded, confident individual who thrives working within a team environment.   You have strong interpersonal skills and have a positive and collegiate approach when working in a team and liaising with customers, co-workers, and stakeholders. You have a robust work ethic and high level of resilience. You are empathetic, insightful, and able to listen without judgement.   You are genuinely committed to a whole of life approach to the support needed for people to live more independently and work towards achieving their goals.  Above all you understand the need to empower our customers, rather than limit their potential.

Please note that you must be able to meet the following criteria:

  • Certificate IV in Disability or Community Services
  • Hold a valid Victorian Driver’s Licence
  • A current National Police Check and Working with Children’s Check
