Located on the Mornington Peninsula, Mentis Assist (Mentis) has been providing quality person-centred, recovery-focused, strengths based, programs for people with diagnosed mental illness & their carers for over 28 years. Mentis also provides HACC eligible clients with Planned Activity and Access and Support Programs.
Mentis has a strong reputation for being an ’employer of choice’ within our region, and as such we are excited to offer this position to experienced applicants who share in the Mentis vision. An opportunity has become available for a motivated HACC Practitioner (working in groups with older clients) to join our dedicated and enthusiastic team. The HACC Program aims to provide effective, person-centred and evidence based service delivery for people who are HACC eligible and may also experience mental health issues, dual diagnosis or other disabilities. Working within a multi-disciplinary team, the successful candidate will have well developed practice skills in recovery focused case management, and an understanding of Diversity planning and the Active Service Model.
Mentis Assist requires its HACC Practitioners to have qualifications and experience in aged care, mental health, disability or a related discipline. Minimum level Diploma or Cert IV with experience. Excellent administrative and IT skills are essential.
