Job Description

An opportunity has arisen for a Palliative Care Counsellor Caseworker to join our friendly interdisciplinary team, providing home-based psychosocial-spiritual care to people living at home with a terminal illness.

This four day a week position involves working with issues associated with dying, death and bereavement, in the following areas:

  • Assessment and care planning
  • Counselling and client advocacy
  • Participation in client care meetings
  • Participation in clinical supervision
  • Opportunity for group facilitation
  • Autonomous management of caseload
  • Ongoing education and learning opportunities

To be successful you will need to have:

  • A degree in Psychology, Social Work, Counselling or Pastoral Care
  • Proven track record in working with families, including culturally diverse
  • Capacity to work with internal and external stakeholders
  •   Maturity to work both independently and within a team
