Home and Community Care Practitioner – Part time, 2 positions (temporary position for 2 days per week for 26 weeks, and permanent equivalent to 4 days per week)
Located on the Mornington Peninsula, Mentis Assist (Mentis) has been providing quality person –centred, recovery focused, strengths based programs for people with diagnosed mental illness and their carers for over 30 years. Mentis also provides HACC eligible clients with Planned Activity and Access and Support Programs.
Mentis has a strong reputation as an employer of choice and as such we are excited to offer this position to experienced applicants who share in the Mentis vision. Two opportunities have become available for a motivated HACC practitioner to join our dedicated Seniors Program team working in groups with older clients. The HACC Program aims to provide effective, person- centred and evidence based service delivery for people who are HACC eligible and may also experience mental health issues, dual diagnosis, or other disabilities. Working within a multi-disciplinary team, the successful candidates will have well developed skills in recovery focused case management and an understanding of Diversity Planning and the Active Service Model.
Successful applicants will have minimum level Certificate IV qualifications in aged care, mental health, disability or a related discipline, excellent IT and administrative skills, plus significant experience in aged care, mental health, or disability.
