An exciting opportunity exists for an Enrolled Nurse to become part of the South Gippsland Adult Community Mental Health Team. Applications are sought from Enrolled Nurses for the position of Intensive Complex Care Coordinator.

The role is to provide time specific, intensive community based support using assertive outreach approach for people who would normally require admission to hospital and during periods of acuity. The coordinator would provide support to people who have been recently discharged from acute bed based services with the purpose of assisting individuals to have a successful transition and discharge back to their communities. This will be targeted at those who are at risk of readmission.

The role is to complement and be adjunct to the community clinician role which will retain full care coordination responsibilities for the consumer.
